Hours of Operation
Times are subject to change without notice. Any situations leading to closing during normal working hours will result in every attempt to post on the website and email campaign. Staff members will unlock the facility 10 minutes prior to opening time for teams to prepare for their usage. Staff will give last call to leave 10 minutes after scheduled closing time and lock the doors no longer than 15 minutes after scheduled closing time. Anyone postponing these procedures without prior approval from South Jefferson SportsPlex Management will be subject to removal of the facility and banning from further use. Staff may close the facility any time after 7:00 pm if no rentals are scheduled. Phone calls in advance are recommended.

General Rules of the South Jefferson SportsPlex
– ABSOLUTELY NO FOOD OR LIQUIDS ARE PERMITTED ON THE PLAYING SURFACE AT ANY TIME.
– No cleats are allowed on the playing serface. Turf shoes and flats shoes are permitted only.
– NO shoes worn outside are allowed to be on the playing surface. All players are asked to change shoes prior to beginning their sessions.
– South Jefferson SportsPlex is a tobacco-free campus. This includes: vaping, e-cigarettes, and smokeless tobacco.
– Members must present their membership cards each time they use the front desk.
– Absolutely no one is permitted to enter the playing area without staff present.
– Only players part of the facility usage activity are permitted to enter the playing surface after facility staff or coaches have given clearance to do so by facility reminder services
– Any member lending his/her membership card to another person will lose all membership privileges. No refund will be given.
– Children age 5 and under may use either the boys’ or girls’ restrooms if accompanied by a parent or guardian.
– Children 8 years and under may not be left unattended in the lobby areas or other parts of the facility at any time unless they are a participating member of the activity.
– South Jefferson SportsPlex is not responsible for lost or stolen items. Please don’t leave valuables in plain sight.
– Prices, fees, facility schedule, hours of operation, classes, and instructors are subject to change. Changes may occur during a session. Notices will be posted when changes are made when possible.
– Please report vandalism or repairs needed so we may maintain the facility in the best possible condition.
– The use of any device capable of photographic images is strictly prohibited in SouthJeff SportsPlex restrooms and changing areas.
– Any use of photo capable devices in program areas must have approval of instructor and facility management.
– To ensure the safety of our members and guests, South Jefferson SportsPlex uses video surveillance and recording throughout the building except in restrooms.
– South Jefferson SportsPlex prohibits staff from babysitting members and guests whom they meet here at South Jefferson SportsPlex.
– South Jefferson SportsPlex may use and take photographs as needed to be used in marketing and promotional materials. Anyone entering the building or grounds may be a part of those photos.
When using the SportsPlex facilities, you must present your membership card. If your card has been lost, a new card can be purchased for a $5 fee. If you forget your card, you will be admitted after personally checking in with the attendant at the front desk. A one day pass may be issued for a lost or forgotten card up to 3 times per year. After 3 times, access to the facility will be denied until a new card is purchased.
Individual Club/Academy refunds will not be given. Cancellations must be submitted in written or in person at least 3 days prior to automatic payment processing.
No refunds are offered for rentals. We offer a voucher for any cancelled facility rental as long as it’s at least 48 hours in advance. Any cancellations or no-shows inside 72 hours will be charged full price.
The South Jefferson SportsPlex reserves the right to cancel any rental or session that does not meet minimum enrollment. Members will be offered a voucher for any items cancelled by the SportsPlex if the cancellation is occurring due to a SportsPlex issue. Any cancellations of sessions or rentals due to a member violation of terms or conditions will not result in a refund or voucher of any kind. Special circumstances may exist and SportsPlex management have the right to alter these terms at any time without written notice.
Membership will be put on a HOLD status for two months and will automatically resume along with fees on your ‘normal’ date,
You can extend a 2 month HOLD, 1 time prior to the resuming of your regular fee schedule but must do so at least 72 hours prior to the date to avoid any fees charged.
As with extensions, all cancellations must also be at least 72 hours in advance to avoid fees (50% monthly fee).
In the meantime, you will continue receiving all club communications via email and TeamSnap.
All changes to accounts must be done in writing via handwritten or email only. No other forms of account changes are accepted for liability purposes.
Non-Sufficient Funds Procedure
If your check/ACH Draft or Credit/Debit Card auto withdrawal is returned unpaid, it will be collected electronically and you will be assessed a minimum fee of $25.00 (or the maximum amount allowed by law). Check writer/Card holder is also responsible for all other collection costs.
Holidays: South Jefferson SportsPlex is closed on designated holidays throughout the year. In addition to scheduled holiday closings, we will have early closings on or around other holidays throughout the year. Such postings will be available on the site calendar.
Inclement Weather: Please visit the website at www.southjeffsportsplex.com or call (315) 804-2665 for information regarding SportsPlex program and facility operations on inclement weather days. South Jefferson SportsPlex will close according to safety regulations, for a 30 minute period after the last lightning flash on all outside activities. South Jefferson SportsPlex will make every effort to make up classes missed due to inclement weather and other SportsPlex closings.
South Jefferson SportsPlex may take and use photographs as needed to be used in marketing and promotional materials. Anyone entering the building or grounds may be a part of those photos.
Standards of Conduct
The foundation of South Jefferson SportsPlex is the mission to build strong kids, strong families and strong communities. South Jefferson SportsPlex creates an atmosphere of a caring community and a place in which everyone is made to feel special and cared about. The standards of conduct have been put into place to ensure that everyone at South Jefferson SportsPlex feels this way.
– Members, program participants or guests who violate one of the standards listed below may have their membership privileges suspended or terminated.
– Vandalism or malicious destruction of SportsPlex or member property
– Lending membership card to a person to gain access
– Possession, use or distribution of drugs or alcohol on SportsPlex property or at SportsPlex sponsored events or programs
– Use of profanity in South Jefferson SportsPlex facility
– Disruptive behavior
– Verbal, threatening or physically or emotionally abusive behavior to other members or staff
– Disrespectful behavior to members, guests or staff including any reference to race, gender, or sexuality.
– All apparel (lack of apparel) should be appropriate for a family environment.
SportsPlex Sex Offender Policy
It is the policy of South Jefferson SportsPlex does not allow a known registered sex offender on any SportsPlex properties or sites (including non-owned sites that we operate) so the safety of participants can be maintained.
WIFI Terms & Conditions
By using South Jefferson SportsPlex of Frederick Guest WIFI internet service, you hereby expressly acknowledge and agree that there are significant security, privacy and confidentiality risks inherent in accessing or transmitting information through the internet, whether the connection is facilitated through wired or wireless technology.

Question: Do I have to be a member to rent your facilities?
Answer: No. Simply call us up and we book your request over the phone or even in person. No membership required.
Question: Do you have a refund policy?
Answer: Yes. Refer to the details under the ‘General SportsPlex Policies’ above. Short of it, 72 hour cancellation required for any refund consideration.
Question: Can I try the club before signing up?
Answer: We’re sorry to say no. Due to liabilities and training ratios, we require all participants to be registered members. What we do recommend is to show up and watch a session and ask all the parents, players, and coaches any questions you have. You will see why we’re the largest and fastest growing club in the area.
Question: I’ve seen your teams playing at the YMCA. How do I join one of them?
Answer: All club teams are required to be active, participating members. Simply register for the club, start training, then qualify for the teams. Depending on the team placement, a minimum practice attendance is preferred and current club dues required.
Question: Do I have to be a member to rent batting cages?
Answer: No. The batting cages are open to the public for rental purposes. Certain waivers and rules are strictly adhered to however.
Question: Do I have to provide my own equipment?
Answer: At this time, due to COVID restrictions, we do not currently have equipment to borrow.
RELEASE AND WAIVER OF LIABILITY AND PARENTAL CONSENT AGREEMENT
In consideration of being permitted to participate in any way in the activity (“Activity”) at South Jefferson SportsPlex, LLC, on behalf of myself and my personal representatives, assigns, heirs, and next of kin, that I

Hereby release, discharge, and indemnify South Jefferson SportsPlex, LLC, their officers, directors, agents, employees and volunteers, other participants, any sponsors, advertisers, and, if applicable, owner and lessors of premises (“Releasees”) on which the activity takes place from all liability, claims, demands, losses, or damages caused or alleged to be caused in whole or in part by the negligence of the Releasees including all expenses and costs incur as a result of any claim.
2.) Acknowledge, agree, and represent that I understand the nature of the activities and that I am qualified, in good health, and warrant that if at any time I believe conditions to be unsafe, I will immediately discontinue further participation in the Activity.
3.) Fully understand that the activities involve risks and dangers of serious bodily injury including permanent disability, paralysis and death; these risks and dangers may be caused by my own or other’s actions or inaction’s, and/or the condition in which the Activity takes place or the negligence of the Releasees named that I fully accept and assume all such risks and all responsibility for losses, costs, and damages that may incur as a result of my participation or that of the minor in the activity.
4.) Understand that at this event or related activities that I may be photographed, and that I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the event holders, producers, sponsors, organizers, and assigns.

PARTICIPATION RULES & REGULATIONS
Please observe the following rules:
Players must wait for the Coach or Instructor to be present in order to enter the playing field(s).
All players bags and equipment are to be kept on the outside of the fence.
NO CLEATS ALLOWED when inside the SportsPlex
No kicking or bouncing the balls outside of the playing fields.
No chewing gum or spitting on the fields.
No food or drinks on the fields.
No Climbing on the fence or equipment.
All protective equipment is mandatory for players AND coaches during sessions.
Children under the age of 10 must be accompanied by a guardian or parents when not on the playing surface..
No alcohol, drugs, or any illegal substances permitted in the facility. If you are suspected of being under the influence of any of these, you will be asked to leave the facility.
No swearing or using foul language.
Persons involved in physical altercations on or off the playing fields will be asked to leave the facility. No running or horse playing in the spectator area.
No pulling or hanging on Goals and/or Nets.
No skateboards, roller blades, scooters or skate shoes.
Must have a shirt and shoes on during sessions..
No intentional use of equipment to hit the ceiling, lights, cables, air movers, etc. Anyone suspected of trying to do so will be removed from the playing area.
The SportsPlex employs teenagers to manage the building in the absence of the owner. Any disrespect or mistreatment of these employees will result in the offender(s) being removed from the facility and potentially banned from future usage.

By completing the registration for new membership, you also agree to read and abide to the following:
During COVID restrictions, you will RSVP in advance and complete a Health Check for every training session your child is qualified to participate in. Failure to do so may result in your child not being allowed to participate.
Follow all SportsPlex COVID guidelines (which are subject to change without notice depending on the current protocols being set by NYSDOH, Jefferson County Public Health, and the South Jefferson SportsPlex
ALL membership cancellations MUST be submitted in writing via email or in person to a SportsPlex staff member and must be done at least 72 hours prior to membership fees drafting your account in order to avoid processing fees.
Absolutely no refunds will be given.
Failure to follow these rules will result in the loss of privileges to participate in the activity by the player and may be grounds for dismissal from the facility.

The South Jefferson SportsPlex system collects information about Registrants on behalf of the organization (“Customer”) in which a Registrant is participating, for use by the Customer. South Jefferson SportsPlex acts as an agent in collecting information from Registrant for the Customer and produces a variety of reports and notifications for the Customer about Registrants. The Personal Information we collect on the site includes first name, last name, email address, mailing address, birthdate, grade, medical information (e.g. allergies, doctor name, etc.), and any information the Customer may choose to collect. Personal Information is protected by a password that Registrant chooses. Users should assure that the password selection is not obvious (name, address, etc.) and should change it periodically. Personal Information is stored in EZFacility TMS, which supports industry standard security. The Database is hosted by EZFacility, a top-grade hosting. EZFacility uses the latest technologies available when ensuring data security and database availability. Registrants can add their photograph, primarily for Customer administrators and/or coaches to identify participants, or to facilitate the printing of a photo-based identification card. If a Registrant is uncomfortable with a photograph as part of their account information, they should delete it (or not add one).
Customer administrators have the ability to create and manage “Teams”. A Team is defined as a group of Registrants, typically referred to as a “Team Roster” or “Roster” that are co-participants during a sport season. For example, a soccer Team might consist of 15 players and 3 coaches/assistant coaches, who are all considered “Team Members”.
Teams: Public Information
The Personal Information that may be shared publicly for the purpose of generating a Team Roster is last name, first initial, Team name and jersey number. Besides the basic Team Roster information, unless otherwise specified herein, other Personal Information collected is not shared with any party other than the authorized representatives of the Customer, unless we are required to do so to complete a requirement for a Customer. For example, Registrant information may be sent as part of a Team Roster submission process from the Customer to Customer’s parent organization(s). Registrants are advised to check with their organization regarding the use and privacy of Personal Information collected on behalf of the Customer.

Updating/Correcting Personal Information
For South Jefferson SportsPlex account holders and end users, Personal Information collected (e.g. name, address, phone numbers, etc.) can be updated, deleted or modified by logging into your South Jefferson SportsPlex account and clicking on “Contact Info” at the top of the page. At any time, Registrants can modify their Team Sharing settings such that no additional Personal Information (with the exception of first and last name) will be shared with Team Members.
Cookies and Tracking Technologies
Session ID Cookies
We use session ID cookies to make it easier for you to navigate our site and to track behavior of users that visit the site, for example, to track pages a user visited to help determine what content is most valuable. A session ID cookie expires when you close your browser.
Our Web pages contain electronic images known as Web beacons (sometimes called single-pixel gifs) and are used along with cookies to compile aggregated statistics to analyze how our site is used and may be used in some of our emails to let us know which emails and links have been opened by recipients. This allows us to gauge the effectiveness of our customer communications and marketing campaigns.

As is true of most web sites, we gather certain information automatically and store it in log files. This information includes internet protocol (IP) addresses, browser type, internet service provider (ISP), referring/exit pages, operating system, date/time stamp, and clickstream data. We do not link this automatically-collected data to personal information.

Credit Card Information
When Registrants use the South Jefferson SportsPlex system to make registration payments via credit card, the payment information is entered on a secure web page, using SSL encryption and processed via a merchant account. Only the last 4 digits of the credit card number are stored in the South Jefferson SportsPlex system. The Customer name (e.g. South Jefferson SportsPlex) will appear on the Registrant’s credit card statement. The South Jefferson SportsPlex system is PCI Compliant. South Jefferson SportsPlex is enrolled in OpenEdge Commerce program, a third party service provider, to validate compliance with the Payment Card Industry Data Security Standard (PCI DSS) mandated by all the major credit card associations including: American Express, Diners Club, Discover, JCB, MasterCard Worldwide, Visa, Inc. and Visa Europe. Trustwave’s Trusted Commerce designation indicates that South Jefferson SportsPlex protects credit card and order information in accordance with payment card industry best practices.
When Registrants/Visitors use the South Jefferson SportsPlex website to donate money, the information will be used by South Jefferson SportsPlex and LeagueAthletics.com solely for the purpose of completing the transaction you have requested and providing customer service and with respect to that transaction. Financial Information such as credit card number and expiration dates will not be stored or saved on our servers.
Absolutely no credit card information is shared, sold or distributed to any third parties. It is used only for the sole purpose of completing the transaction you requested.
Social Security Numbers
South Jefferson SportsPlex uses a service with SportsEngine for thorough background checks on all staff and coaches call Risk Management, to help manage the background check process for individuals designated by our Customer. While it is understandable that consumers are reluctant to disclose their Social Security Number, it is a crucial piece of information to help ensure the name being provided will match potential criminal records, to acquire a list of possible aliases used, and to provide additional information reduce the number of false positives resulting from “common name” matches. Social Security numbers are captured as part of a Registrant’s profile, and passed on to our background check partner (via secure communications), who performs the background check. The specific background check provider used is fully disclosed during the background check registration process. The Social Security numbers are deleted from the South Jefferson SportsPlex system once the results are returned (often minutes after the user supplies the information).
We occasionally post customer testimonials on our web site, in social media, or news cycles, which may contain personal information such as the customer’s name. We do obtain the customer’s consent prior to posting the testimonial to post their name along with their testimonial. If you wish to update or delete your testimonial, you can contact us at firstname.lastname@example.org.
E-mailing Registrants is the primary method by which Customers communicate with Registrants – and a vital component of the South Jefferson SportsPlex system. Therefore, Registrants can expect to receive periodic e-mails from the Customer regarding sign-up deadlines, meeting notices, and other timely notifications.
E-mail information collected is not shared with any other party, unless authorization is granted and explicitly defined in South Jefferson SportsPlex’s Service Agreement with our Customer.
We reserve the right to disclose your personal information as required by law and when we believe that disclosure is necessary to protect our rights and/or comply with a judicial proceeding, court order, or legal process served on our website. If South Jefferson SportsPlex is involved in a merger, acquisition, or sale of all or a portion of its assets, you will be notified via email and/or a prominent notice on our Web site of any change in ownership or uses of your personal information, as well as any choices you may have regarding your personal information.
If you use a bulletin board, blog, or chat room on this web site, you should be aware that any personal information you submit there can be read, collected, or used by other users of these forums, and could be used to send you unsolicited messages. We are not responsible for the personal information you choose to submit in these forums.
